MONROE IT’S TIME TO RUN CLUB WAIVER

MINOR PARTICIPANT AGREEMENT • ACKNOWLEDGMENT OF RISKS • PARENT/GUARDIAN CONSENT

1. Activities and risks

I understand my child may participate in running, conditioning, hills, drills, stretching and related club activities at tracks, parks, roads, trails and other locations. Risks include falls, uneven terrain, traffic, collisions, weather, heat or cold illness, dehydration, overexertion, sprains, fractures, cardiac events, serious injury and, rarely, death. Not all risks can be eliminated.

2. Consent, health and safety

I consent to my child’s voluntary participation. To the best of my knowledge, my child can participate safely subject to restrictions I disclose to the club. I will communicate relevant medical concerns and emergency information. My child will follow safety instructions, stop activity for concerning symptoms and report injury or illness. I understand this is not a physician’s medical clearance.

3. Parent/guardian release and limits

To the extent permitted by Michigan law, I release Coach Todd Williams, Monroe It’s Time to Run Club, and its staff and volunteers from claims that I personally may lawfully release arising from my child’s participation, including claims based on ordinary negligence to the extent such release is lawful. I understand that signing does not waive my child’s independent legal claims where Michigan law does not allow a parent to do so. This agreement does not release claims that cannot legally be waived.

4. Emergency assistance

I authorize club staff to call 911 and seek emergency medical assistance for my child when reasonably necessary, including if I cannot be reached. I understand I may be responsible for resulting medical costs as provided by law. Club staff are not required to provide medical treatment.

5. Scope

This agreement applies to Monroe It’s Time to Run Club activities during the program. If any provision is unenforceable, the remainder will apply to the extent allowed by law. Michigan law governs this agreement.
